Public-sector allowances on top of basic pay

Shift allowance

~5% of basic salary for three-shift rotation.

Night-duty allowance

~15% of basic per night shift worked.

Weekend allowance

~10% of basic per weekend shift.

Rural allowance

8% extra for nurses at scheduled rural hospitals.

Scarce-skills allowance

Public sector (OSD)

R12,500 – R63,000/mo

Department of Health employed nurses. Pay set by OSD; GEPF pension and medical aid subsidy.

Private hospital (Netcare, Life, Mediclinic)

R20,000 – R45,000/mo

Higher base + shift differentials. Provident fund and medical aid usually subsidised.

Agency nursing

R38,000 – R55,000/mo if full-time

No employer; nurses are paid per shift. Higher gross but no benefits and irregular work.

Clinic and GP rooms

R15,000 – R28,000/mo

Steady hours, lower pay. Common for older nurses or those wanting weekday-only schedules.

Occupational health (mining, manufacturing)

R30,000 – R55,000/mo

Strong pay and benefits, day shifts. Competitive entry — usually requires occupational nursing diploma.

Cruise ships / overseas

Equivalent of R60,000 – R120,000/mo

High gross (USD/EUR-denominated), tax-friendly. Long contracts and away from family.

Specialisation pays off

A Professional Nurse who completes an advanced diploma in critical care, theatre or midwifery can earn 15–35% above base. Public sector pays scarce-skills allowances; private hospitals add a specialty premium directly to base pay.

How much does a Professional Nurse earn in South Africa in 2026?+
In the public sector under the OSD, a Professional Nurse Grade 1 (entry to 9 years' experience) earns between R25,400 and R34,200 per month. Grade 2 (10–19 years) reaches R42,100, and Grade 3 (20+ years) goes up to R49,200. Private hospital base salaries are similar but with higher shift differentials.
What's the difference between a Professional Nurse and an Enrolled Nurse?+
A Professional/Registered Nurse holds a 4-year Bachelor of Nursing (or earlier R425 diploma) and is registered with SANC. An Enrolled Nurse holds a 2-year qualification and works under the supervision of a Professional Nurse. The salary gap is typically R8,000–R12,000 per month.
How much does private hospital pay compared to public?+
Headline pay in private (Netcare, Life, Mediclinic) is similar to OSD Grade 1–2, but the take-home tends to be higher because shift differentials (15–35% night and weekend) are paid generously, and overtime opportunities are more frequent. Public-sector pension benefits are usually stronger.
What is the OSD?+
The Occupation-Specific Dispensation is the public-sector pay framework introduced in 2009 for nurses. It links pay progression to years of registered experience plus performance. The DPSA reviews it periodically — most recent COLA was the April 2026 4.0% adjustment.
Can I do agency nursing on top of a permanent job?+
Yes, with restrictions. Public-sector nurses generally need their HOD's written permission and must not work more than 30 extra hours a month. Private-sector contracts vary — most don't permit competing with the employer.
How much does specialisation pay?+
