Rural Allowance (Inhospitable Areas)

10–18% of base for doctors at scheduled rural facilities. Often includes accommodation.

Scarce Skills Allowance

~22% extra for specialty areas chronically short-staffed (anaesthesia, family medicine in rural areas).

One month's base salary, paid in birth month.

Housing allowance

Private practice — what doctors actually take home

Gross billings in private practice are higher than public-sector salary, but expenses are heavy. Real take-home is usually 45–60% of gross practice billings:

Take-home is after rooms rent, support staff salaries, medical indemnity insurance (R30k–R200k/year by specialty), HPCSA fees, ICD-10 software, PAYE / provisional tax. Solo vs group practice changes the maths considerably.

The community-service bottleneck

There are not enough community service posts for all qualifying doctors. In 2024–2026, hundreds of new doctors faced delays of 6–12 months between completing internship and securing a CommServ post — meaning no pay during that gap. National Department of Health acknowledges the problem; budget constraints have prevented immediate fix.

How long does it take to become a specialist in SA?+
A minimum of 13 years from matric: 6 years MBChB, 2 years internship, 1 year community service, then 4 years of registrar training plus CMSA exams. Sub-specialty (e.g. cardiology after general medicine) adds another 2–4 years.
Do doctors earn more in private than public?+
Generally yes for specialists, often no for medical officers. Private specialists running their own practices commonly earn R200,000–R600,000+ per month gross, but pay their own admin, indemnity, premises and staff. Junior doctors in the public sector earn more than equivalent-experience private locums once overtime and benefits are counted.
What is commuted overtime?+
Public-sector doctors above intern level can sign a "commuted overtime" contract — fixed monthly overtime added to their base salary, in exchange for working an additional 16, 24 or 32 hours per week. It can add 30–60% to gross pay.
What is the rural doctor allowance?+
Doctors placed at scheduled rural and deep-rural facilities receive an Inhospitable Areas Allowance of approximately 10–18% of base salary, plus accommodation in many cases. The allowance is partly designed to retain doctors after their compulsory community service year.
Are doctor salaries increasing in 2026?+
Yes. The DPSA Circular No. 15 of 2026 confirmed a 4.0% cost-of-living adjustment effective 1 April 2026 for OSD medical professionals. Annual notch progression continues on top.
How much do GPs in private practice earn?+
A South African GP in private practice typically grosses R900,000–R2,500,000 per year, with the median around R1,400,000. After premises, staff, indemnity insurance and SARS, take-home is usually 45–55% of gross.
